Application for UP Manila Alumni ID

UP Manila Alumni ID Application Instructions:

Thank you for your interest in applying for the UP Manila Alumni ID. To ensure a smooth application process, please carefully follow the instructions below:

Requirements:

1. Accomplished Application Form: Please fill out the attached application form completely.
2. 1x1 ID Picture: Provide one recent 1x1 ID picture with white background.
3. Official Receipt: You have two options for paying the alumni ID fee:
a) Pay the alumni ID fee (Php 150.00) at the UP Manila Cash Office. Present your accomplished application form during payment.
b) Pay via Linkbiz. Please refer to the attached payment procedures for detailed instructions.
4. 1 Valid Government-Issued ID: Submit a clear copy of one valid government-issued ID selected from the following list for verification purposes:
-Philippine Passport
-PhilSys National ID
-Social Security System (SSS) Card
-Government Service Insurance System (GSIS) Card
-Unified Multi-Purpose Identification (UMID) Card
-Land Transportation Office (LTO) Driver’s License
-Professional Regulatory Commission (PRC) ID
-Philippine Postal ID (issued November 2016 onwards)

Once you have gathered all the requirements, please submit them via email to upm-alumni@up.edu.ph. Make sure to include the following in your email:

1. Official Receipt/Confirmation Receipt: Attach a clear photo or scanned copy.
2. High-resolution 1x1 ID Picture: Attach the digital file of your ID picture.
3. Accomplished Application Form: Attach the completed application form.
4. 1 Valid Government-Issued ID: Attach a clear photocopy.

After receiving your complete requirements, the Office of Alumni Relations (OAR) will verify your application. Once verified, the OAR will proceed with processing your ID. Please allow some time for this process.

You will receive an email from the OAR once your ID is ready for pick up. Kindly await this confirmation before visiting the designated location for ID collection.
